Transitional Cell Carcinoma of the Ovary


Abstract
Transitional cell carcinoma(TCC) has been only recently recognized as one of the main types of ovarian carcinoma. The absence of benign Brenner component separates TCC from malignant Brenner tumor. TCCs are sufficiently distinctive to warrant
their
separation from both Brenner tumors and poorly differentiated ovarian carcinomas because the ovarian carcinomas containing predominant TCC pattern have an excellent response to chemotherapy and improved patient survival.
We report a case of ovarian TCC which occurred in a 49-year-old female. Grossly, the right ovary measured 7¡¿5.5¡¿3 cm and showed a poorly circumscribed reddish gray friable tumor. Histologically the right ovarian tumor closely resembled TCCs of
the
urinary tract with thick, blunt papillae with smooth luminal boarders and fibrovascular cores that extended into cystic spaces. The tumor cells were moderately differentiated and showed squamous differentiation, especially at the superficial
portion of
the tumor and infiltrative nests within the stroma. Immunohistochemically, the tumor cells were positive for cytokeratin and epithelial membrane antigen. Carcinoembryonic antigen was positive at the area of squamous differentiation. The patient
showed
multiple metastases within the pelvic and abdominal cavities at the time of operation, and their histopathologic findings were similar to those of the right ovary Although chemotherapy was performed after operation, she expired 6 months later.
